HIPAA Forms

The San Antonio (Texas) Bar Association and the Bexar County Medical Society have approved forms which they deem in compliance with the Health Insurance Portability and Accountability Act of 1996.  Prior to the approval, there were hundreds of unique medical authorizations just in Bexar County which lead to widespread hassle and confusion.  See Jeffrey C. Two forms were approved:

• Authorization for physicians and healthcare providers
• Authorization for hospitals and healthcare facilities
