A report by the General Accountability Office has discovered errors in Social Security Administration death records that resulted in payments to dead people that should have gone to the living.

The GAO believes the inaccuracies resulted in $108 billion in improper payments made in 2012.  The report warned that the Social Security Administration risks using erroneous information because it does not verify death reports from sources such as state vital records departments.
